Supported Housing Programs

Overview

Yamhill County Health and Human Services (YCHHS) strives to support individuals with behavioral health conditions to reside in the most independent living environment possible. At times, individuals with challenges and barriers attributed to serious and persistent mental health conditions experience barriers to obtaining and maintaining community tenure. Services and supports provided in home and community settings are thought to promote improved quality of life, and increased community integration and housing stability. It is the goal of YCHHS to support individuals in achieving positive, desirable housing and community tenure outcomes through our supportive and supported housing treatment programs. Residents of many of these sites may receive wrap-around support from Yamhill County Adult Behavioral Health. A residential team coordinates with individuals' case managers to provide a vast array of services to assist residents in maintaining community living. Services include medication monitoring, conflict resolution skills, emotional regulation skills, money management, symptom management, and help with cooking and grocery shopping, as needed.


Bridges

  • 7-unit crisis and respite-supported housing site. It is designed to assist people in gaining or maintaining stability in their mental health by providing short-term, wrap-around support.
  • Our crisis units serve individuals who need to regain mental health stabilization through medication management.
  • Our respite clients are provided with a safe, supportive place to practice symptom management skills and learn new skills.
  • The referrals for Bridges come from our Crisis Assessment Team and they determine whether a client meets eligibility criteria or not.
  • Many of these individuals are currently enrolled in Adult Behavioral Services but some who come to us, are not. If they are not receiving services from us, we quickly work on getting them enrolled in services.
  • Length of stay is from a day or two and up to a month. We have one unit designated for longer term which is 18 months.

Baker Field

  • 7-unit transitional housing apartment complex. It provides housing for individuals with mental illness who require support and skills training to be successful with independent living.
  • We have a Peer Housing Specialist who lives on-site and in conjunction with the residential team, provides daily on-site support and skills training as needed.
  • Referrals for Baker Field come from our Behavioral Health therapists, case managers, and peer supports.
  • Length of stay is up to 12 months and all individuals are granted Section 8 Housing upon move-in.

Aspen Ridge

  • 15-unit apartment complex that services individuals with significant co-occurring health and medical challenges who want to improve their health outcomes.
  • This supported housing site requires abstinence from illicit substances and marijuana although it does not require abstinence from alcohol.
  • Residents who meet the income requirements are granted Section 8 Housing upon move-in.
  • We have an on-site Peer Support Specialist and in conjunction with the residential team, provide daily on-site support and skills training as needed.
  • Length of stay is 1 year to permanent housing.

Sunnyside

  • 15-unit clean and sober apartment complex for individuals with mental health challenges and who require ongoing support with independent living tasks.
  • Residents who meet the income requirements are granted Section 8 Housing upon move-in.
  • Our residential team provides daily onsite support as needed for medication assistance and training for the development of symptom management, communication, and independent living skills.
  • Length of stay is 1 year to permanent housing.

Homeport

  • 14-unit apartment complex which provides long-term housing for individuals with mental illness and who require ongoing support with independent living tasks.
  • Residents who meet the income requirements are granted Section 8 Housing upon move-in.
  • Our residential team provides daily onsite support as needed for medication assistance and training for the development of symptom management, communication, and independent living skills.
  • Length of stay is 1 year to permanent housing.

Reed House

  • A three-bedroom, single-family home owned by a community member.
  • YCHHS provides support with referral and placement and may provide on-site support as needed.
  • The lease agreement is held between the tenant and the homeowner/landlord.
  • In the event of room vacancies, YCHHS provides reimbursement of the rent to hold the room until the vacancy is filled.
  • This is a clean and sober living environment.

Deskin Commons

  • An integrated apartment complex in which eight units are reserved to provide long-term housing for individuals with mental illness.
  • Residents who meet the income requirements are granted Section 8 Housing upon move-in.

Rental Assistance Program (RAP)

  • Fifteen slots for providing mental health services and financial assistance to support individuals with serious mental illness (SMI) to become ready to rent, locate and obtain housing, and maintain stable, integrated housing after placement.
  • A Residential Housing Coordinator and Peer Support Specialist are available to support all RAP participants to assist with overcoming barriers to securing and maintaining housing.
  • Provides short-term monthly rental assistance of up to $867 per month until individuals can secure permanent rental assistance (i.e., Section 8 Housing), or achieve self-sufficiency. Participants are required to pay 30% of income toward monthly rent.
  • Eligibility criteria includes diagnosis of Serious Mental Illness and must meet one of the following priorities: transitioning out of Oregon State Hospital or licensed residential setting, at risk of reentering a licensed residential or hospital setting without housing support, homeless, or at risk of becoming homeless.

Forensic Rental Assistance Program

Five slots to provide the same RAP services described above with additional criteria of having current/past criminal justice involvement and are at risk of recidivating.

Fast Track Section 8 Vouchers

  • Section 8 Housing Subsidy granted to clientele of Yamhill County Health and Human Services that allows individuals to bypass the Section 8 Housing waitlist.
  • Through a competitive application process, Yamhill County HHS was awarded up to eight Section 8 Fast Track Vouchers per month (HAYC discretion, based on funding)
  • Recipients of Section 8 Vouchers are selected on a case-by-case basis and are based on individual needs and barriers.
