Immunizations

We are now accepting appointments for flu vaccines. Appointments are available Tuesday through Thursday, starting in October. Please call our office at 503-434-7525 to schedule. We accept Providence, Moda, and OHP (Medicaid) insurances.

Required Immunizations

State law requires that children in attendance at public and private schools, preschools, childcare facilities, and Head Start programs in Oregon receive certain immunizations, or have appropriate exemptions in place (OAR 333-050-0010 to 0140).

All children who are not up-to-date or complete on their immunizations or children who do not have a medical or non-medical exemption will be excluded from their school or childcare facility.

See more information about medical and non-medical exemptions.
